Report Form Training

You may have heard about 4-H members receiving trips to Washington, D.C., 4-H Sea Camp, Leadership Camp, or receiving 1 of 80 possible awards, but didn't know how they were selected. Come to the November 16th training to find out.

The Report Form also known as the Ohio 4-H Achievement Record Form is a detailed summary of 4-H accomplishments.  It has categories such as 4-H projects, 4-H program participation, leadership, citizenship, a 4-H Story, and a photo page.

Training on completing the report form will be held on Monday, November 16, 2009 at 6:30 p.m. at the Extension Office, 1802 Princeton Road, Hamilton.

If you want to bring your laptop, feel free.  You will be able to type along as we go through the report form.  CD's with the report form and hard copies will be available at the training.

Please call the Extension office at 513-887-3722 by November 13th and let us know if you are coming so that we can have adequate materials and handouts.
