Forms & Instructions for the 2026 Ohio 4-H Achievement Record (for 2025 activities)

NEW!     Starting in 2026, annual Achievement Record forms are due by January 20th for county or state awards. Forms submitted in the fall of 2025 will be considered for this deadline. You may submit a new or updated form by January 20, 2026 to be considered for county awards.  (Fall submissions were required to be considered for the 2026 Washington Focus Trip awards. 2027 Washington Focus Trips applications will be due next fall)

There are two forms, one for awards issued from the state and one for awards issued from the county. The forms are identicaly except for the cover sheet. Members should complete two forms if they want to be considered for both State and County awards. 

All forms must be printed and delivered to the OSU Butler County Extension Office by the deadline. Deliver to 1802 Princeton Road, Suite 400, Hamilton, OH, 45011. There is a dropbox outside the building that is accessible 24/7. If using the dropbox, place reports in plastic bags to ensure they do not get wet from rain. 

Complete the State Award form to be considered for awards issued by the state. State awards are issued to members age 14-18 as of January 1. Click Here to see more information about state awards.
